Appointments
All office
visits are by appointment only.
All co-pays
are due at the time of service.
If we do
not participate with your health insurance all payments are expected at the
time of service.
So you may
better understand your financial responsibility we encourage you to check with
your insurance company before your visit to verify coverage for preventative visits
as well as deductible or co-pays for conditions/complaints, diagnostic testing,
etc.
We ask our
patients to bring along a list of the medications they are currently taking
and/or prescribed by other physicians. Please include name, dose and frequency.
Be sure to include vitamins, herbal
treatments, and food supplements as well.
Prescription Refill Policy
Please be
prepared with a list of medication prescription refills you will need at the
time of your routine office visit. Since refill requests are handled during
office visits, we do not refill medications over the phone.
Please request long-term medication refills
during office visits with your doctor.
Missed Appointment Policy
Our
practice strives to provide not only the finest medial care, but also to
provide a high level of efficiency and customer service.
Your time is as valuable as ours.
In order to have adequate office hour
coverage, and to keep on schedule during our office hours, we need our patients
to arrive on time for their scheduled appointments and give us adequate notice
when they are unable to keep a scheduled appointment.
We require 24 hours notice for cancellation
of an appointment. Three or more missed appointments, without adequate notice
notice, in a 6 month period may result in dismissal of the patient from the
practice. We regret the need to act on such a policy, but do so in the best
interest of all the patients we serve.
Insurance Referrals
We require
72 hours notice for managed care insurance referral requests.
Please provide us with the specialist or
facility, the date of your appointment, diagnosis or condition, and your
insurance.
Most referrals are done
electronically except for Cigna, Geisinger, and Amerihealth Mercy.
For these insurance referrals, you will need
to pick up the written referral at our office. You will not hear back from us
unless we have questions regarding your referral.
