Appointments

 

All office visits are by appointment only.

 

We have same day appointment availability for acute illnesses if you call us by 10:00 AM. You will receive an automated appointment reminder call generated by Televox, an automated voice messaging system. If you prefer not to receive a reminder call, please notify the office staff.

 

We request that patients arrive promptly for all appointments.

 

We require 24 hour notice for cancellation of appointments.

 

Before being seen for a visit, we encourage you to check with your insurance company to verify coverage for preventative visits as well as deductible/co-pays for conditions/complaints, diagnostic testing, etc. so you may better understand your financial responsibility.

 

All co-pays are due at the time of service. If we do not participate with your health insurance all payments are expected at the time of service.

 

Bring all of your medications that you are taking, including medications prescribed by other doctors, when you come in for your routine office visit. If you cannot bring the medications with you, prepare a list that includes the name, dose, and frequency of all medications. Be sure to include vitamins, herbal treatments, and food supplements as well.

 

Prescription Refill Policy

 

Please be prepared to know the medication prescription refills you need at the time of your routine office visit. It is not our policy to refill medications over the phone. Refill requests are handled during office visits only if you continue under our active care. Please request long-term medication refills during office visits with your doctor. No routine prescriptions will be refilled after hours or on the weekends.   

 

 

Missed Appointment Policy

 

Our practice strives to provide not only the finest medical care, but also to provide a high level of efficiency and customer service.   Your time is as valuable as ours.   In order to have adequate office hour coverage, and to keep on schedule during our office hours, we need our patients to arrive on time for their scheduled appointments and give us adequate notice when they are unable to keep a scheduled appointment.  

 

We require 24-hours notice for cancellation of an appointment.   Three or more missed appointments, without adequate notice, in a 6-month period may result in dismissal of the patient from the practice.   We regret the need to act on such a policy, but do so in the best interest of all the patients we serve.

 

Insurance Referrals

 

We require 72 hours notice for managed care insurance referral requests. You may call directly to our referral line to make your request at 402-0103. Please provide us with the specialist or facility, the date of your appointment, diagnosis or condition, and your insurance. Most referrals are done electronically except for Cigna, Geisinger, and Amerihealth Mercy. For these insurance referrals, you will need to pick up the written referral at our office. You will not hear back from us unless we have questions regarding your referral.
